  How to write an affidavit for Certificates correction.

One of the valuable documents is certificates which we acquire after many years of hard study.
Sometimes our that documents are wrongly written by the authority or sometimes it is written mistakenly by ourself.Like our name is correct according to our National Identification Number but our name is falsely written at out valuable certificates then we all need to correction our certificate with an affidavit signed by the Notary Public.

Here is the information about how to write a name amendment affidavit.

Steps to write:

1. Give information about yourself (according to the national identification card, like full name, addresses, nationality, etc.).
2. Say (do hereby solemnly affirm) and declare as follows:)
3. Start with that and say, I am a permanent and law-abiding citizen of Bangladesh, and the attached photo is mine.
4. Start with that and say about your certificates issued authority who had given your certificate including your roll number, registration number,passing year etc.
5. State about your actual name and how wrongly written(wrong name and right name you want to correction.
6. Is it better to write a table including previous name and actual name. 
7. Declare the statement is true to the best of your knowledge. 
8. Signature of the deponent.
