How to write an affidavit for declaration of a father's and mother's birth and death.
It's very important to have a birth and death certificate for every person. But if it is not registered at the authority file, then the authority is not able to give that certificate if you have no documents.
If someone's birth date or death date is February 2, 1939, then it's impossible to have a birth certificate, and any person in his family needs to declare his birth date or death date on behalf of him with an affidavit.
Here I am going to provide all information related to this affidavit.
Steps to write :
1. Give information about yourself (according to the national identification card, like full name, addresses, nationality, etc.).
2. Say (do hereby solemnly affirm) and declare as follows:)
3. Start with that and say, I am a permanent and law-abiding citizen of Bangladesh, and the attached photo is mine.
4. Start with that and say about your father and mother's birth dates and their death dates. Say your relationship with them.
5. State their death cause and place.
6. State where they were buried (burial ground).
8. Say about—why their information is not written. (Like wrongfully and mistakenly writing their information in the government register.).
9. Is it better to write a table including name, father's and mother's name, birth and death date, cause of death, and relationship with them?
10. Declare the statement is true to the best of your knowledge.
11. Attach a passport-size photo.
12. Signature of the deponent.
13. Signature of Advocate.
14. Print out in 300 taka stamp.
