How to write a name amendment affidavit.
In our daily lives, we face many issues if our name is wrongly written in our academic certificates as well as our national ID card and other documents.
If someone's actual name is (Ahmed Rahim), but his name was wrongfully written as (Ahmed Rahim Khan) in his academic certificates or other documents, then he should correct his name via an name amendment affidavit.
Here I am going to share information about this affidavit.
Need to follow some steps to write this :
1. Addressing the authority, like the Notary Public (who has the power to sign the affidavit).
2. Start (with That).
3. Explain in which documents or information are real (If NID documents are real, then write full name with full address) and add (do hereby solemnly affirm) and say as follows:)
4. Say (that I am a law-abiding and permanent citizen in Bangladesh)
5. Explain in which information your name is wrongly written. (Start with that).
6. Explain which information you need to correct. (Start with That.)
7. It's better to give a table below in which information is wrongly written and which information you need to correct.
8. Say the information is correct to your knowledge (like that the statements made herein above are true to the best of my knowledge, belief, and truth, whereof I signed this AFFIDAVIT before the Notary Public, Sylhet, on this the ...th day of .... 2025.
9. Signature
10. Then say about DEPONENT
(The deponent is known to me and identified by me who has signed in my presence.)
10. Then Signature of the Advocate.
11. It's better to attach a passport-size photo to the affidavit.
12. Printout with Stamp(300 taka Stamp).
